COURSE SCHEDULE

CHAPTER 1 - Performance Variability

In Chapter 1, we examine performance variability, the limitations of relying on incident-based learning, the work failure rate, and interesting similarities behind reasons for both failures and successes.

CHAPTER 2 - Why People Don't Follow the Rules and Use Workarounds

Chapter 2 examines the various reasons why people often don’t follow rules and resort to workarounds. The chapter explores different constraints that contribute to this behaviour, including individual, managerial, and operational factors.

CHAPTER 3 - The Gap Between Plans and Reality and How Teams Depend on Each Other

Chapter 3 focuses on analysing the disconnect between plans and reality, and the interdependence of teams. We explore the concepts of “work as imagined” vs “work as done”, highlighting how teams rely on each other. Additionally, we take a look at the idea that assigning blame cannot be attributed to just one person, emphasising the role of dependencies in complex systems.

CHAPTER 4 - Your Beliefs Determine What You See

Chapter 4 discusses attribution errors and biases in accident causation, emphasising the idea that, in general, we tend to find what we’re looking for. The chapter explores how our preconceived notions and biases can affect our understanding and analysis of accidents.

CHAPTER 5 - Does Holding People Accountable Lead to More Incidents?

In Chapter 5, we explore the side effects of punishment, psychological safety, accountability frameworks, types of harm, restorative justice’s effectiveness, and just culture frameworks. Through Kate’s story, we examine how these concepts intersect and impact personal growth, team dynamics, and the pursuit of justice and accountability.

CHAPTER 6 - Do Your Questions Shut People Down?

Chapter 6 focuses on the importance of avoiding the “WHY?” question and instead using open-ended questions, specifically TEDS questions, to facilitate open and meaningful conversations with employees. We examine the background theory and then run through specific examples to bring the concept to life in a practical manner.

CHAPTER 7 - Walk-Through / Talk-Through (WTTT)

Chapter 7 focuses on leadership engagement for learning and provides insights on where to start. We introduce the Walk-Through/Talk-Through (WTTT) method, explaining how it differs from a “safety” conversation. Continuing Kate’s story, we showcase WTTT in action and highlight common mistakes to avoid when conducting WTTT sessions.

CHAPTER 8 - Learning Teams

Chapter 8 provides a deeper look at the Learning Teams process, covering phases one through three, and finishes with practical, real-world examples.

CHAPTER 9 - Integration and Scaling-Up

Chapter 9 recaps everything we’ve learned in previous chapters and shows practical examples of how you can implement LFNW in your organisation – from setting a roadmap to scaling it across your organisation for increasing results.
