HOW IT WORKS
Our Learning Transformation course is a 4-session workshop designed for safety leaders to help envision how organisational learning can be transformed through the lens of modern safety.
You’ll explore the enablers of success to transform learning at the strategic and tactical levels from a needed mindset shift to integration with existing processes.
Building on multiple schools of thought and combined with business strategy, participants will learn a different way of thinking about failure and how to translate it into a fresh safety strategy that prioritises learning before, during, and after accidents by listening to employees’ voices on difficulties and challenges.

Based on multiple schools of thought including Human and Organisational Performance (HoP), Human Factors, Safety 2, Engineering Psychology and many others, this session will introduce key concepts and how they apply in the real world. You will practise applying these concepts to your operations.

FROM INCIDENTS

Root cause analysis is not the most effective tool to explain human and organisational behavior. This session, informed by systems thinking and modern accident causation models, will provide a practical introduction to how failure happens in large organisations and the practical tools to learn more effectively from failure.
The session will cover:
1. The limitations of cause and effect to explain human and organisational behavior
2. How failure happens in complex systems
3. Things go wrong for the same reasons that things go right
4. Outcome equivalence – why addressing …causes of accidents does not prevent a similar accident in the future
5. How misalignment between different stakeholders affects learning (HR, legal, ops, safety)
6. Biases that distort how we see causes
7. HoP, HF and Safety 2 tools for incident investigations
8. Communicating findings – How to tell a story about the context to reduce the risk of blame

MODERN ACCOUNTABILITY
AND RESTORATIVE JUSTICE

Leaders and organisation’s response to failure directly affects ability to learn.
This session, based on the psychology of justice, psychological safety, and restorative practices, will transform how you think about just culture and recovery of trust after failure.
The session will cover:
1. Does accountability equal punishment?
2. Who draws the line on what is acceptable?
3. How leadership response to failure can prevent effective learning
4. Hidden side-effects of punishment
5. Introduction to restorative justice
6. Evidence of effectiveness
7. Types of harm and how different people are affected
8. Alternatives to punishment to drive positive accountability
9. How to restore trust and rebuild relationships

PRACTICAL TOOLS
AND INTEGRATION

This session will demonstrate how to apply various tools in practice and how to integrate the concepts and techniques with the existing processes.
The participants will develop their own learning transformation plan to start their journey.
