How MeiraGTx integrated error traps into their risk assessment process.
The situation
MeiraGTx is a pharmaceutical company based in London.
Their risk assessment process, like most in the industry, focused on identifying hazards and assigning controls.
The deeper factors that influence whether people interact with those hazards safely, such as confusing equipment design, procedure gaps, and time pressure, were not being captured.
What was delivered
Psychology Applied delivered workshops bringing together senior leaders from safety, quality, and operational teams.
The sessions covered:
- The psychology behind hazard identification and risk perception
- The difference between hazards and error traps
- Dynamic risk assessment skills for shop-floor teams
What were the outcomes
The team worked collaboratively through improvements across six areas of their risk assessment ecosystem:
- Policies and procedures
- Templates
- Software integration
- Training programs
- Coaching and quality control
- Data and KPIs
What was said
A thought-provoking day with practical solutions to include error traps and human factors in our risk assessments.
What made this work
The workshop produced a practical plan for integrating error traps into MeiraGTx's existing risk assessment infrastructure, not as a separate layer but as part of the process teams already use. The cross-functional participation (safety, quality, and operations working together in the same room) meant that the plan had ownership from the people who would implement it.
